Living Well: Integrated Care 'Pioneer' Project
Supporting independence for vulnerable people-Social movement rather than healthcare strategy
NHS Kernow’s ‘Living Well’ programme was endorsed by the UK department of Health in November 2013 when it was awarded integrated care ‘pioneer’ status; one of 14 sites in the country.
In response to an invitation from the HSCB, the leaders of the programme will visit Northern Ireland on 6th November. We are delighted that they have agreed to deliver a seminar on their work in Cornwall.
The leadership team comprise of:

- Dr Tamsyn Anderson, Urgent Care Lead and Newquay Project Lead
The pilot demonstrated saw significant improvements to wellbeing, fewer emergency hospital admissions with previously dependent people now offering support to their peers. The pilot demonstrated a change in the way services are delivered with front line teams voluntarily coming together and developing a new way of working based on respect for each other’s expertise.
Living Well takes the learning from this pilot and, with voluntary sector funding and full support from NHS Kernow, Cornwall Council, HealthWatch Cornwall and all key providers, aims to support and measure the impact on 1,000 people living in West Cornwall.
All parts of the system are working together to measure the impact on our three main outcomes: improved quality of life, improved experience of care and reduced cost of care. Success was measured using the Institute of Healthcare Improvement’s ‘AAA’ methodology which included looking at things from the patients experience, safety and quality of services and how to best use resources.
The INVITE
The HSC Leadership Centre would like to invite HSC colleagues to this short session to update colleagues on the key developments in Integrated Care in the NHS in England.
We are delighted to be joined by the Managing Director of NHS Kernow, Joy Youart, who is a highly experienced, nationally recognised, successful change leader in service integration.
She led NHS Kernow through the first wave authorisation and is now leading the organisation.
The “Living Well” vision and strategy has been nationally recognised through an HSJ award and is one of the 14 selected ‘Pioneers’ on integration.
Joy is passionate about service improvement, real grass roots engagement and developing new collaborative cultures and relationships across Cornwall, which is enabling joined up services for the people of Cornwall.
